A document with the name of the infectious disease and the period of suspension of attendance may be substituted.(III) Obtain, fill out, and submit a “Notification of Absence due to Quarantine for Infectious Diseases” to the office of your affiliated department (undergraduate/graduate) and follow all instructions given by the office.
